I consent to the processing of my personal data for purposes related to the organization, operation and implementation of the training service. The administrator of personal data is Grupa Active Change Leszek Zawlocki, ul. Powroźnicza 6/2, 30-307 Krakow. The data is processed on the basis of art. 6 sec. 1 lit. b of the regulation on the protection of personal data of April 27, 2016 (GDPR) and in accordance with the Act of August 29, 1997 on the Protection of Personal Data and the Act of August 26, 2002. on the provision of electronic services. The collection of this data is voluntary, but necessary to provide the service. The data will be stored for up to 6 years based on accounting regulations or longer based on art. 17 point 3 sec. e Regulation of the European Parliament and of the EU Council 2016/679 of April 27, 2016. At any time, you can request access to your data, correction, rectification, deletion, processing restrictions, transfer of personal data and permanent deletion (except for situations provided for by law). Acceptance by the Active Change Group of data from this form does not constitute participation in the course. The confirmation of willingness to participate is the payment of an advance payment of PLN 500 to the account of the Active Change Group, which is both the recruitment fee and the reservation of the place. We reserve the right to select people for the School of Gestat Practitioners in Organizations on the basis of an interview. If you do not receive confirmation from us within 2 weeks, please contact us again (the e-mail may have been stopped by anti-spam filters). Resignation from participation is possible no later than 14 days before the start date of the training program. In the event of cancellation less than 14 days before the start of the program or in the event that the participant does not show up for the training, he/she bears the full cost of the program.
